Requirements
A family wishing to enroll a child in BHPS must have proof of the child's satisfactory completion of the grade in which s/he was previously enrolled and must also provide evidence of the child's ability to achieve at the grade level in which s/he is applying. This is accomplished via a two-day "shadowing" period accompanied by class-administered tests and a final evaluation. Upon admittance, there will be a mandatory 4 1/2 week academic/behavioral probationary period; and, should academic and/or behavioral expectations fail to be met, a parent conference will be held to discuss the student's status at the school.
Required Documentation:
- Documentation of successful completion of previous grade
- Copy of most recent report card
- Any pertinent academic/behavioral test results
- Copy of birth certificate
- Florida Department of Health Certificate of Immunization
- Florida Department of Health Form DH680 (blue)
- Florida Department of Health Form 3040 (yellow)
Along with the above, all necessary BHPS enrollment forms (found here) must be completed and returned to the school.
Additional Requirements:
